Navigation is much of the non-clinical work required to push through barriers to care. This covers a long tail of activities; in our work so far, we’ve found some major buckets of needs to include:
(1) medication access (e.g. finding patient assistance programs)
(2) insurance benefits usage (e.g. finding an in-network provider)
(3) supportive referrals (e.g. for transportation grants, social services, home care, smoking cessation, nutrition counseling, support groups etc).
Much of the learning is done on the job; we think the key skill required to succeed in navigation is problem-solving. We will have weekly team huddles where we discuss barriers our respective patients are facing, and we will work through them as a team.
We will provide some sources to start, and over time, we expect to build up a database of resources and information that we can use for each new patient we onboard.
The role is fully virtual. We expect navigators to make a minimum number of patient calls in order to make a dent in patient care at our partner practices, and we will have a weekly or biweekly touch-base with other navigators/us. We are asking that navigators commit to a minimum of 5 hours, and based on fit and capacity, we may offer navigators the option to scale up from there.
